Orbital decompression surgery is crucial for treating severe exophthalmos and optic nerve compression in thyroid-associated ophthalmopathy (TAO) patients. Traditional techniques often pose risks such as postoperative diplopia and periorbital numbness due to extensive manipulation of orbital structures.

 

This study introduces a modified deep lateral wall decompression technique that strategically removes bone at the sphenoid door jamb while preserving surrounding anatomy. Results demonstrated significant improvements in visual acuity, exophthalmos, and intraocular pressure, with a notably low incidence of complications. This promising surgical approach suggests a safer option for TAO management, warranting further exploration through larger, controlled trials.
