Integrated management of cortisol: A multidimensional perspective

Xia & He Publishing Inc.

Cortisol dysregulation drives metabolic, psychiatric, and inflammatory disorders. This review synthesizes evidence on integrative strategies—nutrition, sleep, exercise, mind-body therapies, and adaptogens (Withania somnifera, Rhodiola, Panax)—that restore HPA balance. Complementary modalities (acupuncture) show adjunctive benefit. An integrated model combining biomedical, lifestyle, and traditional approaches offers evidence-based pathways for stress-related disease management.

Introduction
Cortisol, regulated by the HPA axis, follows a circadian rhythm. Chronic dysregulation (hyper- or hypocortisolism) underlies metabolic, psychiatric, and inflammatory diseases. Integrative management combines pharmacotherapy with lifestyle and mind-body interventions.

Key Strategies

  • Nutrition: Low-glycemic diets, omega-3s, magnesium, vitamin C reduce cortisol and inflammation.

  • Sleep & exercise: Consistent sleep, morning light, and moderate exercise restore circadian rhythm and reduce HPA overactivation.

  • Mind-body: Yoga, meditation, MBSR lower cortisol, IL-6, TNF-α.

  • Adaptogens: Withania somnifera (most evidence; 27.9% cortisol reduction), Rhodiola, Panax, Ocimum, Bacopa modulate HPA and enhance resilience.

  • Acupuncture: Reduces cortisol and inflammatory markers.

Safety
Glycyrrhiza (licorice) causes hypertension; Withania may affect thyroid; high-dose omega-3 and melatonin have risks. Herb-drug interactions via CYP450 require caution.

Conclusions
Cortisol regulation requires integrative, personalized approaches combining nutrition, sleep, exercise, mind-body practices, and evidence-based adaptogens. Future research should use validated biomarkers (salivary, hair cortisol) to guide precision interventions.
