HDL-free cholesterol (FC)spontaneously transfers to other phospholipid surfaces on a time scale of minutes. This schematic illustrates the consequences of FC transfer from FC-poor (Top) and FC-rich HDL (Bottom). The latter transfers more FC to than the former to other lipoproteins and the plasma membranes of cells, including macrophages. This difference may explain the higher all cause and cardiovascular mortality in patients with very high plasma HDL concentrations.
